How do I submit payment?Training price is $3200. Payment is due at booking. Once your application has been submitted you will be redirected to a payment page to complete your registration. An invoice will be provided for your records at checkout.

What is the training schedule?Training will begin at 9:00am with introductions. Didactic training will be held from 9:15am to 12:15pm. This will include consultation & assessment, facial mapping, product rheology, and implementation. Trainees will be provided models in order to apply learned consultation, tools, and create individual treatment plans with trainers. Lunch break will occur between 12:30pm to 1:15pm. Lunch will be provided. Trainees will be divided into 2 small groups and live injections will begin at 1:30 pm. -Group 1 will participate in Sarah Berg`s injection and Group 2 will participate in Nina Burdett`s injection training between 1:30pm-3:30pm -Group 2 will participate in Sarah Berg`s injection and Group 1 will participate in Nina Burdett`s injection training between 3:45pm-5:45pm.

Do I have to be a medical provider to participate?Yes, all trainees must submit an active RN, NP, PA, MD, or DO license to participate in this medical training.

How many people will be a part of the training?In order to provide an intimate setting for learning participants will be limited to 8 trainees.

Where is the training?Training will be held onsite at Forma Medical Aesthetics & Wellness. Our office is located at 11780 Northfall Lane Unit 301 Alpharetta GA 30009

Will I be injecting?We will offer 2 types of training options. The first for observation only. The second will be for trainees to inject their own model. On site injecting does require additional insurance coverage for trainers and site visit. In order to inject: You must have a valid medical GA license to inject You must provide your medical directors full name and licensing. Provider must provide an active malpractice policy for underwriter review. This includes insurance company and declaration page only. Injectors will provide their model. Models will be prescreened by trainers no later than Wednesday July 3, 2024. All product and supply will be included and provided to injectors at training.

How long is the training date?The training will be held on July 12, 2024 from 9am to 6pm.

What will be taught at the training?Injection training will cover both on label and off label techniques for full face and neck rejuvenation. This will include dermal filler, skinboosters, biostimulator injections, Sculptra, neurotoxins, and regenerative medicine techniques and therapies.

How long does it take Dysport® to work?Though results vary, over half of users see improvement in moderate to severe frown lines between the brows within 2-3 days.

What is Dysport® used for?Dysport® is a prescription injection administered by a licensed healthcare professional to treat moderate to severe frown lines (those wrinkles between the eyebrows). Medically, it’s a botulinum neurotoxin (abobotulinumtoxinA), an acetylcholine release inhibitor and a neuromuscular blocking agent. It temporarily prevents specific muscle movements that cause frown lines.

What are Dysport® injections?Administered by a licensed healthcare professional, Dysport® is supplied as a single-use, sterile 300-unit vial. The dose of Dysport® for the treatment of glabellar lines (frown lines between your eyebrows) is a total of 50 units injected equally among the specific muscles. The dose of Dysport® is not the same as the dose of any other botulinum toxin product and cannot be compared to the dosage of any other botulinum toxin product that you may have previously used.

Will you provide products?Yes, we will provide all product for your models at a discounted rate. Trainee may also choose to bring their own products.

How long is a shadow day?Shadow days are from 9am to 4:30pm Tuesday through Thursday.

Will you provide models?Trainees are responsible for securing models, submitting a good faith exam, health history, and photos for approval.

Do you travel out of state or to private practices?No, all training is conducted on site at Forma Medical Aesthetics and Wellness.

Is there a set up fee fee?Our Wellness Membership requires a one time set up fee of $1,400.00. Your $200.00 Consult Fee is applied towards this. There are no set up fees with our Aesthetic Memberships.

Is there a minimum commitment for membership?Yes, our Wellness Membership requires a minimum commitment of 6 months and all aesthetic memberships require a commitment of 90 days. You may cancel for any reason with 30 days notice.

How long does it take to get lab results?Lab results take 3-7 business days on average to result. I prefer to use my insurance for bloodwork. We find that even with insurance, our cost is far less. Some panels run through insurance cost as much as $400 dollars. If you prefer to use your own insurance to submit blood work, we provide order details for you.

I am prediabetic. Can the doctor send my peptide prescription to my pharmacy so insurance can cover the cost?Yes, our MD, an internal medicine doctor, well versed with management of diabetes and other metabolic diseases can prescribe Semaglutide for you*. We recommend consultation with your Primary Care Doctor for management for diabetes.

How much is the lab work?Lab work is included in the initial consultation.

How is this different from Phentermine?Phentermine, a central nervous stimulant, increases heart rate and metabolism while decreasing appetite. We do not prescribe any stimulants in our program. Semaglutide does not work on the central nervous system and will not cause feelings of a racing heart, anxiety, or jitteriness.

I have lots of questions, and I am not sure if I want to move forward. Could I just meet with the doctor to discuss my questions?Yes, absolutely! Forma welcomes you to come in and discuss any questions. Our $200 MD consultation fee can be applied to your program should you decide to move forward within 30 days of your consultation.

Will I have to inject myself?Yes, if prescribed injectable peptides, but no if prescribed oral or topical peptides. If you need injections, a small insulin needle works to minimize any potential discomfort. One-on-one training by our nursing staff prepares you for self-administering injections, and we provide you with injectable training pamphlets for reference.

Where will I get my supplies?Your prescribing pharmacy will provide you with the option of syringes, alcohol pads, and sharps containers for disposal. Once full, you deliver your sealed sharps container to our staff for proper disposal.

Do I need to change my diet?Yes, a healthy diet supports your weight loss journey and helps you reach your goals. We recommend a diet high in green leafy vegetables, fiber, and protein. Stay hydrated and manage you water intake. Reduced hunger means you must pay attention to consuming the right number of calories. However, there is no need to count calories, carbs, or macros.

What are the side effects?Dr. Morgan A reviews a complete list of side effects during your consultation. The most common reported side effects, nausea and headache when beginning your program, occur due to the slowed emptying of your stomach. We start you with a low dose of Semaglutide initially and prescribe nausea medication on an as needed basis. As systems subside, we increase your dose. Some patients have also reported they no longer find alcohol appealing.

Why does the source pharmacy matter?Not all Semaglutide is made equally and knowing the source of medication is critical. We only utilize Wells pharmacy for quality sourcing and compounding. Why Wells Pharmacy? PCAB accredited pharmacy (Pharmacy Compounding Accreditation Board). Wells practices the most comprehensive compliance in the industry based on the U.S Pharmacopeia Convention guidelines. They require screening and patents for quality control.

I cannot inject myself. May I come to the office for my injections?Unfortunately, if you are unable inject yourself, we cannot prescribe the program.

How quickly will I lose weight and how much weight will I lose?Weight loss varies and depends on dose. Most patients should expect to lose between 4%-8% of their initial weight monthly. For example, if you weigh 170 lbs., anticipate losing 6.8 to 13.6 pounds per month. Some patients may require 2 months to see this type of weight loss. It does take 3 days for the medication to reach a serum level in the body, meaning a level high enough that the body can recognize and use the medication.

Is bloodwork required?Yes. This is a prescription drug, and a full health history and assessment must be completed prior to treatment to ensure your safety and success.

What if I receive peptides somewhere else, but want to transition to Forma?We require, for your own safety, a complete workup with our MD.

What is the difference between semaglutide, ozempic, wegovy, and tirzepatide?Semaglutide and Tirzepatide are similar in that both work by mimicking the hormone glucagon-like peptide-1 receptor agonist (GLP-1). This hormone helps the body regulate blood sugar levels by signaling the pancreas to release insulin and the liver to decrease glucagon production when the body is in a hyperglycemic state (high blood sugar levels). Both medications delay the gastric emptying of the stomach reducing hunger, regulating the appetite, and increasing feelings of satiety; making it easier to create a calorie deficit and aid in weight loss. Both Semaglutide and Tirzepatide have been shown to reduce cholesterol levels, triglycerides, as well as overall mortality rates in people with type 2 diabetes. In comparison Tirzepatide, the active ingredient in Mounjaro, not only mimics the effects of GLP-1 like Semaglutide but is also a dual glucose-dependent insulinotropic polypeptide, GIP hormone. This means it works by imitating not one, but two unique incretin hormones produced by the human body. Incretins are hormones which are released from the gut into the bloodstream in response to any food we ingest. Among other things, tirzepatides help promote a feeling of fullness by delaying gastric emptying, boost insulin production, normalize blood sugar levels and activate the satiety centers (feelings of fullness after eating) in the brain. Tirzepatide has been shown in clinical trials to both decrease food intake and increase energy expenditure making it a highly effective weight loss medication. Tirzepatide has also been found to lower fasting blood glucose levels and increase weight loss in participants. While Tirzepatide acts on both GIP and GLP-1 receptors, and Semaglutide acts solely on GLP-1 receptors it does not mean that Semaglutide is an inferior medication when it comes to improving metabolic health and aiding in weight loss. How well either medication works for your body depends on a multitude of individual factors ranging from: your bloodwork, inflammatory markers, autoimmune disorders, genetic profiling, medical and health history, physical predispositions, and lifestyle choices. A complete health evaluation with lab work and screening is required for our doctor to help determine the safest, most effective medication and dose for you based on all of the individual factors listed above.

Who is not a candidate?Type 1 diabetics, those pregnant or breastfeeding, patients with thyroid cancer or a history of multiple endocrine neoplasia syndrome type 2, or any patients with a history of pancreatitis. A patient with known allergies to Ozempic or Semaglutide would not be a candidate.

I have recently done bloodwork. Can I provide the results of my lab for review?Yes, upload any blood work completed in the last 60 days to your Patient Now portal. Please be aware that we may still require you to complete additional blood work as many required serum levels are not included in standard blood panels.
